台灣戰後嬰兒潮己逐漸步入中高齡，台灣人口高齡化及少子化趨勢將持續成長。退休人口逐漸回歸社區，增加社區動能與活化社區人口，另一方面也增加社區老人照顧的需求。本課程旨在創造一個理論與實務對話的平台，讓學生能運用社區組織原理、理論與方法的學理概念，以「萬華社區」為實驗社區，以台大北護分院長照系統的運作為「在地學習」的目標，檢視與討論「醫療與長照」組合結構，以及其如何與「在地社區」整合，以支持高齡者的健康與臨終關懷照顧體系之基礎。
本課程將以台北市的萬華區長照2.0 A-B-C模組的服務輸送系統，做為課程討論及學生參與觀察學習的主要實驗場域。希望透過課程的講授、社區協力組織的參與互動，以及進入實驗場域的觀察及參與，思考如何整合在地醫療與社會服務資源，朝向以社區為基礎的整合式照顧服務體系，如何可以從支持家庭、居家、社區到機構式照顧的多元連續服務，達成健康促進、預防失能、以及落實在地老化、在地便利的社區照顧。
本課程分為三大部分：第一部分為介紹社區原理、社區組織的學的基本概念與理論，第二部分是邀請實驗社區中主要的行動者進行說明，其中包括台大醫院北護分院長照系統的各部門，並邀請當地非營利的社區協力組織，進行對話與社區組織的深入暸解。第三部分為學生進入實驗場域，進行社區診斷，試擬以社區為基礎的健康促進或教育介入方案(community-based program)。
Taiwan will become a superaged society in 2025. The ageing population continue to grow in the future. The course is to provide a dialogue between practice and theory with respect to the issues regarding how to provide a community-based social system to provide social care and medical care for ageing population. This course will provide opportunities for students to engage in long-term care-provision issues surrounding a community hospital and its connections with local community organizations. The complexity of working with local community organizations will be addressed in this course. This course is divided into three parts: the first part is to provide students with knowledge and concepts of community and the workings of community organizations. The second part is to provide students with opportunities to observe, and to engage with the long-term care-provision of a community hospital and collaborate with hospital staff to work out their own community-based projects for learning purpose.
